Did you have any miscarriages or problem pregnancies prior to your pregnancy with me?

Did you have any staining, bleeding or other signs of possible miscarriage during your pregnancy with me?

Did you have diabetes or problems of high blood pressure during any pregnancy?

Did you undergo any type of illness, surgery or hospitalization during your pregnancy with me?

Did your doctor ever tell you he was giving you a medication to help you keep the pregnancy?

Did your doctor ever give you special prenatal vitamins that included hormones as "pregnancy insurance"?

Did your doctor ever give you special injections to guard against miscarriage or to stop bleeding or cramping?

Is Dr. ______________ or any of his partners still alive?

Would you request your records of the pregnancy with me from Dr. __________________?
