Do I have any changes or abnormalities of the vaginal tissue (e.g., adenosis)?

Do I have glandular tissue in the vagina?

Do I have any abnormalities of the cervix, such as a hood, cock’s comb, ridge or hypoplastic cervix (smaller than normal)?

Do I have any uterine abnormalities? (These can typically only be determined by hysterosalpingogram – a dye-enhanced x-ray of the uterine cavity – or by hysteroscopy – examination of the uterine cavity by a scope inserted through the cervix)

Do I have a T-shaped uterus? Do I have a hypoplastic uterus?

Do I have any abnormalities of my fallopian tubes? (These are typically found either on HSG or by laparoscopy)
